GRAND CHUTE, WI (WTAQ-WLUK) – OSHA proposed fines of more than $175,000 for Konz Wood Products, after a worker was killed in an industrial incident.
Francis Wolfinger, 57, died in the Dec. 5 incident.
OSHA identified 15 serious and two repeat safety violations during inspections afterwards, with fines totaling $177,453, records show.
Two violations were directly related to the death, OSHA confirmed.
- The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.
- The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6).
The Grand Chute Fire Department says it was called to Konz Wood Products, 616 N. Perkins St., around 3:40 a.m. When they arrived, paramedics found Wolfinger on the floor with traumatic injuries. The fire department did not release more details about his injuries, except to say that he was not entrapped or entangled in any machinery.
